Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint investigation was initiated on June 22, 2020 and concluded on August 25, 2020. The records review for the complaint investigation was combined with the remote monitoring inspection. A complaint was received by the department regarding allegations involving interactions among staff and youth. The Executive Director was contacted by telephone to conduct the investigation. The inspector emailed the Executive Director a list of documentation required to complete the investigation. Interviews were conducted and records and documentation reviewed. The evidence gathered during the investigation did not support the allegations. Exit meeting was held via telephone with the Executive Director on August 25, 2020. The licensee has five calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Please do not post the Supplemental form. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.
