Comments:
A renewal inspection was initiated on September 7, 2021 and concluded on September 14, 2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census was 50. The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed three youth records, four staff records. and background checks for two board officers, submitted by the agency to ensure documentation was complete. An exit meeting was conducted with the Executive Director on September 14, 2021 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation were documented on the violation notice issued to the facility.

Description: Violation:
Based on review of the personnel record for Staff S4 and interview with agency representative AR1, the record did not contain a copy of the educational degree or include dates for the relevant work experience with details substantiating qualifications, as required by this chapter.

Findings:
1. The personnel record for S4 did not include a copy of the educational degree or diploma required by the job description.
2. The employment application and resume for S4 did not include dates and details substantiating the qualifications for the relevant work experience, as required by this chapter.
3. AR1 acknowledged this during the interview with the licensing inspector.

Plan of Correction: Executive Director and Program Director will ensure all new hires/staff have copies of educational requirements in the personnel files. Will also ensure that all new hires/staff have completed up-to-date resumes that clearly outlines the job experience needed.
