Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 9/25/24 from 10:50 PM ? 3:05 PM - Inspection conducted. 9/26/24 from 10:55 AM ? 5:10 PM ? Inspection conducted. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of youth in care: 74 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: 7 Number of staff records reviewed: 8 Number of interviews conducted with staff: 2 Number of interviews conducted and tour of youth apartment: 1 Additional Comments/Discussion: An entrance conference was held at the agency on 9/25/24 with the Executive Director. The Executive Director and the QA Director were available and accessible during the inspection. Staff and youth records were reviewed.. On 9/26/24, a preliminary findings meeting was held with the Executive Director and the QA Director to seek clarification and provide technical assistance. An exit meeting was conducted on 10/2/24 to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
