A monitoring inspection was initiated on 1/12/2022 and concluded on 1/13/2022. The Executive Director was contacted via phone regarding COVID protocol. The inspector made an onsite inspection to the physical location. COVID-19 protocol measures were in place and followed by the licensee. After clearing the COVID-19 questions and protocol and inspection was initiated. The inspector reviewed the current license and the previously posted inspections. The State Trainer and the TFC case manager discussed that there were no agency or program changes during the entrance conference. The Licensing Inspector discussed the current census and requested items required to complete during the documentation review portion of the inspection. The Executive Director reported a current census of Five (5) approved foster care home, Five (5) foster children in care. The Inspector reviewed (1) foster care home, and (1) foster child record was reviewed. (1) Personnel record was reviewed during this inspection. An exit interview was conducted with the State Trainer and the TFC Case Manager and the Executive Director where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined that was (two) violations of non-compliance(s) with applicable standards or law, and there were violations documented on the violation notice issued to the facility. The Licensing Inspector addressed the violations with the Program Director and supervisor who were both available to address any questions. The State Trainer, TFC case manager, and the Executive Director were all available to address any questions.

Based on a review of the record for FC1 and interview with the TFC Case Manager, the licensee failed to ensure that a written quarterly progress summary was completed.
Findings:
FC1 was placed with the licensee on 10/11/2021. The record does not document a completed quarterly progress summary.
