Type of inspection: Monitoring

Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 12/23/2023 onsite visit to the office setting in Norfolk. On 2/27/2024 the record's review was completed from 10:30 to 2:00pm.

Number of children in care: 8

Number of approved provider homes: 3

Number of children?s records reviewed: 1.

Number of provider home records reviewed: 1

No new staff hired in the Norfolk since the last inspection.

Observations by the licensing inspector: positing of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection on 2/28/2024

Standard #: 22VAC40-131-180-J-2
Description: Based on the review of the records presented for Foster Home (FH)-1 and based on an interview with Agency Representatives (AR), the licensee failed to document the date and signatures of the individual who completed the home study and the Executive Director and/or designee.

Findings:
1. Based on the review of the initial home study narrative for FH (1), the documentation did not contain the date and signature of the individual completing the narrative.

2. The review of the initial home study narrative for FH (1), failed to contain the date and signature of the Executive Director or designee.

3. During the exit interview AR-1 acknowledged the absence of date and signatures on the home study narrative

Standard #: 22VAC40-131-250-O
Description: Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with Agency Representatives (AR), the licensee failed to document a written assessment child?s record of immunizations since the last examination.

Findings:
1. Based on the review of the child record for FC1, there was no documented written assessment with 30 days of the child?s placement containing all the elements
specified in 22VAC40-131-250 G.

2. During the exit interview AR-1 acknowledged the absence of the written assessment.

Standard #: 22VAC40-131-290-F
Description: Violation: Based on the record review for FC-1, the agency failed to include documentation from the dental visit for 1/4/2024.

Findings:
1. The monthly home visit report dated 2/1/2024 indicated that a dental follow-up was completed on 1/4/2024, but there was no documentation included in the child's file.
2. The findings were discussed during the preliminary findings and exit review.
