Type of inspection: Monitoring
An unannounced onsite monitoring inspection was held on 11-07-2024 from 10:30 p.m. to 1:30p.m. The entrance interview was also conducted at the time of arrival with agency staff. Additionally, remote inspection days included 11-08-2024, 11-20-2024. An acknowledgement of Inspection form was signed and left at the facility.

The licensing inspector completed a tour of the physical plant that included the office setting.

Evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.
Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-150-B-1 Description: Violation:
Based on record review and interview, the agency failed to provide evidence that all the required orientation training was provided to S1 and S2 within 30 days of employment.
Findings:
1) In review of S1 and S2?s records review on 11-7-2024, the following elements were not identified on the training roster provided by the agency;
? Policies and procedures regarding expectations for service delivery;
? Practices regarding protection of confidential information;
? Documentation protocols;
? The Standards for Licensed Child-Placing Agencies (22VAC40- 131), child-placing related Virginia statutes, and child-placing related policy and guidance documents and broadcasts issued by the Division of Family Services, Department of Social Services;
? Prohibition of corporal punishment;
? The licensee's policies regarding discipline and behavior management;
? Pre-Service training core competencies for resource foster and adoptive family home providers
2) The day of the onsite inspection, 11-7-2024, the licensing inspector contacted S5 via email correspondence to inquire whether the missing training elements were provided to S1 & S2. By the conclusion of the onsite inspection 11-7-2024, the additional training elements had not been presented to determine full compliance with the identified regulation.

Standard #: 22VAC40-131-160-B-5 Description: Violation:
Based on a record review of S3, the agency failed to provide evidence that an annual performance evaluation was completed for S3.
Findings;
1) In review of S3 record on 11-7-2024, an annual performance evaluation was not on file for review.
2) The day of the onsite inspection, 11-7-2024, the licensing inspector contacted S5 via email correspondence to inquire whether the annual performance evaluation for S3 was on file. S5 indicated that ?there has not been a performance evaluation completed yet for S5?.
