Comments:

A renewal inspection was initiated on August 3, 2021 and concluded on August 16, 2021. The Executive Director was contacted to initiate the inspection. The Executive Director reported the current census included seven youth in independent living arrangements, three approved provider homes, and four children in foster care placements. The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed one youth record, one foster home record, one child?s record, and two staff records submitted by the agency to ensure documentation was complete. An exit meeting was conducted with the Executive Director on August 16, 2021 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and a violation was documented on the violation notice issued to the agency.

Description:	Based on review of the background checks for staff S2 and interview with agency representative AR2, the agency failed to ensure the background check (a criminal history record) was completed before three years since the date of the last background check. Findings: 1. Criminal history record checks were provided for review for S2. The criminal history record checks were dated 09/20/2017 and 10/05/2020. 2. During an interview with AR2, AR2 acknowledged the background check was out of compliance.
Plan of Correction:	Executive Director will meet with HR Manager to discuss system of tracking for background checks and make sure all background checks are completed in a timely manner.
