Type of inspection: Renewal An unannounced renewal/monitoring inspection was conducted on-site 07/29/2024 from 10:45 a.m. to 1:00 p.m. The Acknowledgement of Inspection form was signed and left at the facility on the date of the on-site inspection. The exit interview was conducted on the same day as the on-site inspection; 07/29/2024. Number of youth (Independent Living): 12 Number of approved provider homes: 0 The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
