Comments:

An unannounced monitoring inspection was conducted on April 19, 2022 from 10:54 a.m. to 2:35 p.m. The agency reported sixteen (16) youth in independent living arrangements. During this inspection, the inspector reviewed one (1) personnel (staff) record and three (3) records for youth (2 current and 1 discharged record). Agency representative AR1 responded to questions during the inspection. A preliminary findings review meeting was conducted with AR1 on April 19, 2022, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. An acknowledgement form was signed. An exit meeting was conducted with AR1 before leaving the agency on April 19, 2022. Upon the receipt of the violation notice, a plan of correction is requested for the violations. The plan of correction or Description of Action to be Taken should include, as appropriate, the following: the steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; position(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five (5) business days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the VDSS website within five (5) business days.

Description:	Violation: Based on review of the personnel (staff) record for staff S1 and interview with agency representative AR1, the criminal history record check result was not received before 30 days of employment at the agency ended. Findings: 1. Staff S1 was hired on 02/28/2022. The criminal history record check was submitted by the LCPA to the Virginia State Police (VSP) on 03/01/2022. 2. On 03/17/2022, the (VSP) returned the request form to the LCPA. The form was missing the notary seal and an address on the money order was incomplete (missing city and state). 2. The corrections were made by AR1 and the criminal history record check resubmitted to VSP on 03/20/2022. 3. LI interviewed AR1 regarding the criminal history record check findings. AR1 did not provide information regarding contact with VSP as described in 22VAC40-191-60-D. As of the date of the inspection, 04/19/2022, the criminal history record check result was not received.
