An unannounced renewal inspection was conducted on March 8, 2022 from 10:07 a.m. to 1:00 p.m. At the time of the inspection the agency reported ten (10) youth placed in independent living arrangements. During this inspection, two (2) personnel records and one (1) youth record were reviewed. Agency representative AR1 was available to respond to questions during the on-site inspection. Preliminary inspection findings were reviewed with agency representative AR1 on March 8, 2022. Acknowledgement form was signed. Exit meeting was held with AR1 on March 10, 2022. The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.

Based on review of the record for Youth 1 and interview with agency representative AR1, the licensee failed to complete the life skills assessment within the last 90 days prior to accepting the youth for an independent living arrangement.
Findings:
1. A life skills assessment was not completed within the last 90 days prior to accepting Youth 1 for an independent living arrangement.
2. AR1 confirmed that the life skills assessment had not been completed within the last 90 days prior to the youth's acceptance.Plan of Correction: Not available online. Contact Inspector for more information.
