Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 4/2/2024 from 10:30 a.m. to 4:00 p.m. The Licensing Inspector met with the Program Director to discuss the entrance to the inspection. The exit interview was held on 4/2/2024. Number of Youth in care: (12) Number of New Staff reviewed since the last inspection: (9).- This number included the CEO who is listed as the CEO/Director on the Virginia SCC. Number of Youth records reviewed: (1). Number of provider Youth Discharge records reviewed: (1) The licensing inspector completed a tour of the physical plant that included the office setting and conditions Observations by licensing inspector: posting of the license and inspection documents was observed. An acknowledgement of inspection was left with the licensee on the date of inspection. There were no changes made to the policies or procedures since the last inspection. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. The findings were discussed during the preliminary and exit interview.
