An unannounced monitoring inspection was conducted on January 16, 2020 from 10:00 a.m. to 11:53 a.m. At the time of the inspection the agency reports 3 youth placed in ILA. One youth record was reviewed during this inspection. The agency reports no new hires since the last inspection. An exit meeting was conducted with the Director of Independent Living Arrangements prior to leaving the agency to discuss preliminary inspection findings. An acknowledgment form was signed.
