Comments:

A non-mandated self-report inspection was initiated on November 1, 2021 and concluded on November 18, 2021. A self-reported incident was received by the department on October 28, 2021. This inspection focused on intake and admission. The Director of Independent Living was contacted by telephone to conduct the inspection. The licensing inspector emailed the Director a list of documentation required to complete the inspection. The evidence gathered during the inspection did not support non-compliance with standards or law. A violation not related to the self-report but identified during the course of the inspection can be found on the violation notice.

Description:	Violation: Based on review of the record for a youth placed in an independent living arrangement, Youth1, and interview with agency representative AR1, the agency failed to document the requirements of this regulation in the 90 day service plan review. Findings: 1. The 09/09/2021 service plan review did not include the beginning and ending dates of the review period (missing element). 2. AR1 confirmed that this information was missing from the service plan review.
