Discussed the written agreement for participation in the arrangement as it pertains to the youth's responsibility to inform the licensee within a specified time frame, but no later than 72 hours, of any major changes in his situation and serious injuries, illness, or need for surgery as noted in the ?Standards for Licensed Child Placing Agencies?- 22VAC40-131-440.E.7 Discussed background checks as noted in the ?Background Checks for Child Welfare Agencies? - 22VAC40-191-40.D.1.b and 22VAC40-191-40.D.1.c

Comments:

An unannounced monitoring inspection was completed by the Licensing Specialist on 4/19/22 from 9:37 a.m. to 4:18 p.m. and 4/20/22 from 10:00 a.m. to 12:57 p.m. The current census is five (5) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. Three staff records were reviewed. On 4/19/22 and 4/20/22, the Executive Director was available and accessible during the inspection. The Executive Director and the Case Worker/Life Skills Instructor were interviewed on 4/20/22 in regard to the youth record. The Director of Human Resources was interviewed 4/20/22 in regard to the staff records. The Acknowledgment of Inspection form was signed and left at the agency on both dates. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website.
