Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
9/27/22 from 10:13 AM to 5:50 PM and 9/28/22 from 10:05 AM to 1:53 PM
The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Additional Comments/Discussion:
An entrance conference was held at the agency on 9/27/22. On 9/27/22, the Executive Director and the Assistant Director were available and accessible during the inspection. On 9/28/22, the Assistant Director was available and accessible during the inspection. The Vice-President of People, Diversity, and Belonging was interviewed on 9/27/22 while staff records were being reviewed. The Assistant Director was interviewed on 9/27/22 and 9/28/22 in order to provide clarification about several documents reviewed. The Clinical Director was interviewed on 9/28/22 about the professional training provided to students or interns. An exit meeting was conducted on 10/5/22 to review the inspection findings.
The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
