The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Additional Comments/Discussion:
An entrance conference was held at the agency on 6/29/23. The Executive Director and the Assistant Director were available and accessible during the inspection. The Vice-President of People, Diversity, and Belonging was interviewed on 6/29/23 while reviewing staff records. The Assistant Director and the Vice-President of People, Diversity, and Belonging were interviewed on 6/29/23 in order to obtain clarification about specific staff records. The following were interviewed on 6/30/23 in order to obtain additional clarification about specific staff records in relation to the independent living arrangements program: the Vice-President of Programs, the Executive Director, the Assistant Director, and the Vice-President of People, Diversity, and Belonging. A preliminary findings meeting was held on 6/30/23 with the Executive Director and the Assistant Director. Lastly, a virtual meeting was held on 7/19/23, which included the following staff: the Vice-President of Programs, the Executive Director, and the Assistant Director. This meeting focused on specific staff records and job descriptions in relation to this agency?s independent living arrangements program.
An exit meeting was conducted on 7/31/23 to review the inspection findings.
The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.
Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-110 Description: Based on interview with staff and review of foster youth?s, FY1?s, record, the agency failed to comply with indicating the date received for all materials and information.
Findings:
1. Upon review of FY1?s record, the life skills assessment was accepted without indicating the date it was received.
2. Staff, S10, acknowledged the date received was not on the above-mentioned document.Plan of Correction: 1. A date section has been added to the CASEY, which enables a youth to indicate the completion date when finishing the CASEY.
2. If a completed CASEY is received with no date, the Director or Assistant Director will ask the youth or DSS worker to submit a date of completion. If the CASEY is older than 90 days, the youth will be asked to complete a new CASEY. If the CASEY was completed within 90 days, the youth or DSS worker will be asked to write the date of completion on the CASEY to ensure compliance with licensing standards.
Standard #: 22VAC40-131-440-F Description: Violation: Based on interviews with staff and review of foster youth?s, FY1?s, record, the agency failed to develop an individual service plan within 30 days of the date of admission to the independent living arrangement program.
Findings:
1. Upon review of FY1?s record, the youth was admitted on 4/17/23.
2. Upon review of this individual service plan, the author added ?details of goals and objectives with strategies? and signed it on 6/4/23.
3. Staff, S9 and S10, both acknowledged the findings.Plan of Correction: 1. Educate staff about the significance of timely service planning for foster youth and the potential consequences of delays.
