Comments:
An unannounced renewal inspection took place on February 23, 2022 from 10:30 am to 3:45 pm at the office of United Methodist Family Services (UMFS) in Alexandria, Virginia. The agency reports 22 approved provider homes and 13 children placed.

During this inspection, 5 provider home records, 2 children records and 4 personnel records were reviewed. Two staff were interviewed during this inspection.

The Regional Director and Family Systems Coordinator were available throughout the inspection. The Regional Director, Family Systems Coordinator, Associate Vice President of Programs, Associate VP of Human Resource, VP of Strategy and Innovations, and HR Coordinator participated in the exit interview to discuss preliminary findings, which commenced at 3:15 pm on 2/23/2022. An acknowledgement form was left with the Regional Director on 2/23/2022.

There was one citation for violation of the Standards for Child-Placing Agencies.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Based on review of background checks for Staff (S) S1, the licensee failed to ensure the criminal history check and sworn disclosure were completed before three years since the date of the last sworn disclosure, and criminal history check.

Findings:
The most recent sworn disclosure for S1 is dated 2/7/22 and the previous sworn disclosure is dated 12/10/18, which is beyond three years.

The most recent criminal record check for S1 is dated 2/4/22 and the previous criminal record check for S1 is dated 12/17/18, which is beyond three years.
