Comments:

Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 12/13/2022 from 10:10 a.m. to 4:30p.m. The Licensing Inspector spoke with the administrative manager via phone upon arriving at the site. The Licensing Inspector met with the Regional Director to initiate the inspection. The acknowledgement of inspection form was left with the Director and the exit interview was scheduled and completed on 12/14/2022 at 11:30.am. to 11:50 a.m. There were no new staff at the location during this inspection. Number of approved provider homes: (14) Number of children in care: (10) Number of children?s records reviewed: (1). Number of provider home records reviewed: (1) and (1) suspended home. Number of staff records reviewed: (2). Including the Board Officers. Licensee maintains COVID protocol onsite and the Licensing Inspector were asked screening questions along with a temperature check. Observations by licensing inspector: posting of the license and inspection documents was observed. There were no major changes made to the policies or procedures since the last inspection. The Licensing Inspector interviewed the Director and (2) TFC Social Worker staff members. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Description:	Based on a review of the sworn statement for staff person (P1), for whom background checks are required, the licensee failed to fully complete the sworn statement 1. Staff person (P1) sworn statement dated 2/23/2022 did not contain an answer to the form questions #1 through #4. 2. During the exit interview, the Regional Director acknowledged the absence of the completed sworn statement form.
Plan of Correction:	Human Resources Department has reprogrammed the sworn statement form to require full completion by staff before it can be submitted.
