Comments:

An unannounced renewal inspection was conducted on December 14, 2021 from approximately 9:45 am to 2:25 pm at the Farmville office of United Methodist Children's Services (UMFS). An entrance meeting was conducted with the regional director. The regional director was available throughout the inspection. The licensee reported 18 children in care and 10 approved provider homes. The regional director, assistant program director, senior human resources manager, VP of strategy and innovation and associate vice president of programs participated in the exit interview conducted on 12/14/2021 to discuss inspection findings. During this inspection, 1 provider home record and 2 foster child records were reviewed. Background checks for one employee and the corporate board officers were reviewed. One employee was interviewed during the inspection. There was a citation for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on review of background checks for applicants, the licensee failed to ensure sworn statements, search of central registry and criminal history checks were completed before three years since the dates of the last sworn statements, central registry finding and criminal history record checks. Findings: The most recent sworn disclosure for applicant (A) A1 is dated 9/21/18 and the most recent sworn disclosure for A2 is dated 11/29/18, which are beyond three years. The most recent search of the central registry for A1 is dated 10/22/18, which is beyond three years. The most recent criminal record check for A1 is dated 9/21/18 and the most recent criminal record check for A2 is dated 11/30/18, which are beyond three years.
