Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An announced monitoring inspection was initiated on 6/9/2021 and concluded on 6/10/2021. The regional director was contacted by telephone to initiate the inspection. The agency reported 5 children in care, and 7 approved provider homes. One child record, 1 provider home record, and background checks for 1 staff were reviewed. One staff was interviewed during this inspection. The regional director was available for questions throughout the inspection. An exit interview was conducted by telephone on 6/10/2021 at 4:00 pm with the regional director, community based services supervisor, family services coordinator, administrative services manager, and assistant director of quality Improvement and risk management to discuss preliminary inspection findings. There was one citation for violation of the Standards for Child-Placing Agencies. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on review of the criminal record check, email and interview with the regional director, the licensee failed to ensure the criminal record check was kept as required by the background check regulations. Findings: Staff (S) S1 was hired on 12/20/2020. The licensee provided an email from the regional director dated 6/10/2021. The email documents that a criminal record check was completed for S1 on 12/17/20 according to the licensee?s human resources department. The criminal record check provided for review for the inspection for S1 is dated 6/10/2021.
