Type of inspection: Renewal A renewal unannounced mandated inspection was conducted on-site 01/12/224 from 11:15 a.m. to 2:50 p.m. An entrance conference was held via e-mail and telephone with the Administrative Service Manager and brief introductions on site. The preliminary findings review and exit review was held on the same date with the Regional Director and Community Based Services Coordinator. An Acknowledgement of Inspection form was signed on the date of the inspection. Number of children in care: 3 Number of approved provider homes: 4 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: 1 Number of provider home records reviewed: 1 Number of staff records reviewed: 6 Staff interview: 2 The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
