Comments:
An unannounced monitoring inspection was conducted on 08/09/2022 from 9:05 am to 2:30 pm. The Acknowledgement of Inspection forms were signed and left at the agency. A total of 25 children are placed and 22 approved provider homes.

Three children records, 3 provider home records and 1 staff record was reviewed. One employee was interviewed during the inspection.

A preliminary inspection findings meeting was conducted on 8/9/2022 to review preliminary findings. The exit meeting was conducted on 8/16/2022.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Description: Violation:
Based on review of the record for Provider Home (PH) PH3, the licensee failed to ensure the national criminal check was completed within 30 days of a person residing in the home becoming 18 years of age.

Findings:
The face sheet in the record for PH3 documents Other Household Member (OHM) OHM1 continues to reside in PH3.

During an interview with the program manager, it was acknowledged that OHM1 resides in PH3.

OHM1 became 18 years of age on 8/13/21.The national criminal record check for OHMI is dated 11/29/21-beyond 30 days of 8/13/21.

Plan of Correction: 1.Weekly- Continuous review of background checks for incoming families. We will review it during our pipeline meeting.

2.Weekly- The family side team will review our tracking form with all our families? due dates.

3. Monthly- At the beginning of each month send out reminders to families of background checks coming due. Will send it out 6- months prior to the due date.

4. Families with placements will receive file reviews quarterly,

5. Families who are reopening will have a file review before they are able to be reopened.

6. Staff will follow up with families verbally and via email; they will receive confirmation for all appointments a minimum of 30 days prior to the due date.
