Type of inspection: Renewal
An on site inspection was conducted on 1/13/23. An exit interview was conducted on 1/20/23.
The Acknowledgement of Inspection form was signed and left at the facility on the date of the inspection.
Number of children in care: 23
Number of approved provider homes: 18
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
Number of children?s records reviewed: 3
Number of staff records reviewed: 5
Number of interviews conducted with staff: 1 entrance interview with the Program Manager and the Administrative Manager. 1 exit interview with the Utilization Manager, VP of Community Based Programs, Chief Program Officer, Quality Improvement Manager, Associate VP of HR, Senior HR Manager, HR Coordinator, and the Regional Director.
Observations by licensing inspector: The license was posted along with inspection documents.
Additional Comments/Discussion:
The evidence gathered during the inspection determined non-compliances with applicable standards or law, and
violations were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a
plan of correction to indicate how the cited violation will be addressed in order to return the facility to compliance and
maintain future compliance with the applicable standard or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the
standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any
names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit
interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental
information), you may request a review and discussion of these findings with the inspector?s immediate supervisor. To
make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that
serves your geographical area.
Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS
public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department?s inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-210-D Description: Violation: Based on a review of a foster family file (FH-1) and an interview with an agency representative (AR-1), the agency failed to provide annual training to the foster family as required.
Evidence:
1) The most recent documented trainings provided to FH-1 were dated 12/09/21 and 08/13/20.
2) AR-1 acknowledged in an email on 1/24/23 that the training they provide is not always updated in their case management system.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-131-260-B Description: Violation: Based on a review of two children?s records (FC-1 and FC-2) and an interview with agency representatives (AR-3 through AR-10), the agency failed to include elements in the children?s social histories as required.
Evidence:
1) The social histories contained in the record of FC-1 and FC-2 dated 11/29/22 and 11/09/22 did not include elements required under 22VAC40-131-260.B.
2) AR-3 through AR-10 had the opportunity to provide evidence that the social histories for FC-1 and FC-2 contained the required elements in an exit interview on 1/20/23 but they did not.
3) The social history for FC-1 did not include:

22VAC40-131-260.B.6. developmental, educational, and medical history
