Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced monitoring inspection was initiated on 06/29/2020 and concluded on 07/02/2020. The regional director was contacted by telephone to initiate the inspection. The agency reported 18 children in care and 20 approved provider homes. The agency reported no new hires since the last inspection. The inspector emailed the regional director a list of items required to complete the inspection. The inspector reviewed 2 children records, 3 staff records and 2 provider home records. Background checks for three additional provider home records were reviewed. The agency previously submitted Covid-19 policies and procedures and revised policies and procedures.

An exit interview was conducted with the licensing inspector by telephone on 07/02/2020 at approximately 11:00 am with the regional director and community based supervisor. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency.

There was 1 citation for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department's public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on a review of the record for Foster Home (FH) FH2 and FH5, the licensee failed to ensure the criminal record checks were dated no more than 90 days of approval of the provider homes.


Findings:
The record for FH2 contains a criminal record check for adult household member (AHM1) dated 8/5/19. August 5, 2019 is more than 90 days prior to the approval date of FH2. FH2 was approved on 1/27/20

P1 is a single adult provider, approved for FH5. The criminal record check for P1 is dated 9/30/19 and the central registry finding for P1 is dated 10/4/19. September 30, 2019 and October 4, 2019 are more than 90 days prior to approval. FH5 was approved on 1/16/2020.

Plan of Correction: To ensure homestudies are completed within 90 days of the receipt of background checks, once background checks are received the 90 day deadline will be noted/tracked by the Administrative Assistant. This report will be provided to the Director and Family Systems Coordinator on a monthly basis. This report will be monitored until the completion of each homestudy and discussed during supervision with the Director and Family Systems Coordinator to ensure the deadline is met. In addition within 10 days of background expiration the Administrative Assistant will notify the Director and Family Systems Coordinator.
