Type of inspection: Renewal
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
01/18/2023 from 12:00 p.m. to 3:00 p.m. and 3:25 p.m. to 5:30 p.m.
01/19/2023 from 9:15 a.m. to 12:15 p.m. and 12:45 p.m. to 4:40 p.m.

The Acknowledgement of Inspection form was signed and left at the facility.

The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Additional Comments/Discussion: The Licensing Specialist reviewed the agency?s Program Evaluation and Improvement plan (UMFS Strategic Plan FY 23-25) and discussed.

An exit meeting was conducted on 1/30/2023 by Teams Meeting.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-290-F
Description: Violation:
Based on record review, the agency failed to arrange for Child 2 (C2) to receive recommended follow-up medical treatment.

Findings:
1) C1 received a physical exam on 7/22/2022 at which time the medical provider recommended another medical examination.
2) Documentation of the arrangement for and the child?s receipt of the follow up medical examination was not located during record review.
3) The findings were discussed during the preliminary findings review.
4) Staff reviewed the record and were unable to locate documentation that the additional medical examination was arranged by the agency.
5) The Licensing Inspector provided additional time for the agency to located documentation regarding the arrangements for the medical examination.
6) The agency did not provide documentation of the agency?s arrangement for the additional medical examination. Documentation provided was an email, dated 1/20/2022, from C2?s custodial agency stating C2 was scheduled for the follow up medical examination 1/24/2022.

Description: Violation:
Based on record review, the agency failed to obtain before three years since the date of the last sworn disclosures, most recent central registry search, and most recent criminal history search results for Staff 2 and 3 (S2 and S3)

Findings:
1) The record for S2 contained Sworn Disclosures dated 6/15/2017 and 10/27/2022.
2) The record for S2 contained criminal history search results dated 6/27/2017 and 12/15/2022.
3) The record for S3 contained Sworn Disclosures dated 2/25/2015 and 10/3/2022.
4) The record for S3 contained central registry search results dated 4/25/2018 and 11/21/2022.
5) The record for S3 contained criminal history search results dated 3/10/2015 and 12/15/2022.
6) The findings were discussed during the preliminary findings review.
7) Staff reported the agency identified errors in the work of a former staff member regarding background checks and the agency has been working to correct.
8) Staff acknowledged the findings.
