Type of inspection: Renewal
This inspection was initiated by two Licensing Inspectors on 2/13/24 and one licensing inspector on 2/14/24.

Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: On 2/13/2024 the inspection started at 10:30 a.m. and ended 5:45 p.m. Inspection conducted on 2/14/24 started at 10:30 a.m. and ended 4:10 p.m.
The Licensing Inspector met with Regional Program Director on 2/13/24 and the inspection was initiated.

Number of approved provider homes: 20
Number of children?s records reviewed: 3
Number of provider records reviewed: 3
Number of staff records reviewed: 8

Observations by licensing inspector: Posting of the license and inspection documents were observed. A preliminary meeting was conducted on the date of the inspection with the Program Director and the acknowledgement form was left with the program Director following both days of the inspection. The exit meeting was completed with Program Director on 2/14/24 at 4:10p.m.

Technical assistance was provided to the licensee regarding a document in the record received by the provider not including a stamp or written date information was received. by the agency.

The evidence gathered during the inspection determined (1) non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee can submit a plan of correction to indicate how the cited violation(s) will be addressed to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. The department's inspection findings are subject to public disclosure.

By signature the facility representative acknowledges that the inspector reviewed all information found on the Inspection Summary, including areas of standards reviewed, date(s) and time(s) of inspection, technical assistance provided and the comments/discussion section.

Description: Violation:
Based on record review, the agency failed to complete a 90-day monitoring visit for Provider Home 1 (P1).
Findings:
1) P1 home was approved 8/6/2021.

2) The record for P1 documented 90-day visits completed on 6/5/2023 and 11/15/2023. The record for P1 did not contain documentation of 90-day monitoring visit for the month of 9/2023.

3) The Licensing Inspector requested documentation of the 90-day monitoring visit that was conducted 90 days from the visit that occurred on 6/5/2023.

4) The findings were discussed during the preliminary findings/exit meeting and staff acknowledged the findings
