Type of Inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
8/7/24 ? 11:45 PM ? 4:30 PM - Inspection conducted and Program Manager was accessible during the entire inspection.
8/8/24- 3:15-4:30- Staff records were reviewed remotely due to records at another licensed location. Program Manager was accessible via telephone and email.
The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

Number of approved provider home: 14
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Number of interviews conducted with staff during onsite inspection: 1
A preliminary findings review was conducted on 08/7/24 and exit meeting occurred on 8/14/24 via TEAMS.
The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.
Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-191-60-G-1 Description: Violation:
Based on review of record for staff (S4), agency conducted a criminal history check to include fingerprints for staff through another state agency other than VA State Police
Findings:
1. Staff (S4) was hired by the child placing agency on 7/29/24. Sworn statement in record for Staff (S4) was dated 7/12/24.
2. Criminal history eligibility letter reviewed in record for (S4) was dated 7/17/24 and on letterhead from another state agency which also included a file/license number for a service not licensed by VDSS. The eligibility letter in record for staff (S4) "another state agency (not VA State Police)" stated, the above-named Children?s Residential Individual is eligible for employment.
3. Licensing inspector discussed inspection findings with Staff (S2) and Staff (S3) and explained the process for completing criminal history search directly through VA State Police to include fingerprints as part of agency?s hiring process.
4. Staff (S3) asked clarifying questions related to VDSS standards and acknowledged findings and plans to follow-up with agency?s HR department to review agency process for conducting criminal history searches.Plan of Correction: Not available online. Contact Inspector for more information.
