22VAC40-131 ADDITIONAL REQUIREMENTS FOR SPECIFIC PROGRAMS

Comments:
The Licensing Specialist conducted an unannounced complaint investigation in response to a complaint received by the licensing office on November 20, 2019. The complaint investigation was conducted on November 20, 2019, while on-site conducting an unannounced renewal inspection from 10:05 a.m. to 2:57 p.m. for the Forest location. An interview was conducted with Staff S2 by telephone on November 22, 2019. Desk audits to review additional documentation were conducted on November 22, 2019, November 25, 2019, December 13, 2019, and December 20, 2019. The complaint investigation occurred due to an allegation regarding the licensee?s acceptance of a youth for placement in an independent living arrangement. Information gathered during the investigation supports the allegation, therefore the complaint is determined to be valid. Exit meeting was held with Staff S1 via telephone on January 13, 2020.

Description: Based on review of the record for Youth 1 and interview with Staff S2, the licensee failed to gather, review, and document information required by this regulation prior to accepting the youth for an independent living arrangement (ILA) placement.

Findings:
1 When interviewed, Staff S2 reported to the Licensing Specialist that the decision to admit Youth 1 into the ILA was based on conversations with the placing agency representative during several FAPT meetings.
2. S2 was unable to provide any written information gathered or any documentation to demonstrate that information required by 250.G was received and reviewed.

Plan of Correction: ED discussed with Program Manager/Child Placing Supervisor the necessity of gathering all required information prior to placement. ILS will only place youth when appropriate information for placement is gathered. PM will discuss in supervision with all Sites.

Findings:
1. Record for Youth 1 failed to document meeting with the youth face to face at least one time to evaluate the youth's readiness for an independent living arrangement (ILA).
2. S2 reported to the Licensing Specialist that the decision to admit Youth 1 into the ILA was based on conversations with the placing agency representative during several FAPT meetings. S2 was unable to provide any written information documenting a pre-placement interview had been completed.

Plan of Correction: ED discussed with Program Manager/Child Placing Supervisor the requirement of conducting an interview with the prospective youth face to face prior to acceptance. PM will discuss in supervision with all Sites.
