Comments:

An unannounced renewal inspection was conducted on January 24, 2022 from 12:05 p.m. to 3:52 p.m. and on January 25, 2022 from 10:10 a.m. to 3:00 p.m. A desk audit to review additional documentation was conducted on January 27, 2022. The agency reports one (1) child placed in foster care with no new approved foster homes since the last inspection. During this inspection, the inspector reviewed one (1) child?s record, one (1) foster home record, two (2) records for youth placed in independent living arrangements, and four (4) staff records. Agency representatives AR2 and AR3 were available to respond to questions during the on-site inspection. Acknowledgement forms were signed. Preliminary inspection findings were reviewed with agency representatives AR2 and AR3 on January 25, 2022. Exit meeting was held with AR1 on February 3, 2022.

Description:	Violation: Based on review of the record for Youth 1 and interview with agency representative AR3, the licensee failed to maintain confidentiality as required by Code of Virginia ? 63.2-104. Findings: 1. The participation agreement for Youth 1 contained the name of another youth, Youth 3. The participation agreement had been reviewed and signed by Youth 1. 2. The Virginia Code requires that records and information of all child-welfare agencies to or on behalf of individuals shall be confidential information. 3. AR3 confirmed that the name of Youth 3 was included in error in the participation agreement for Youth 1.
