Comments:

A monitoring inspection was initiated on 8/24/2021 and concluded on 8/26/2021. The executive director was contacted by telephone to initiate the inspection. The executive director reported that the current census was 1 child in care and 25 approved provider homes. The inspector emailed the executive director a list of items required to complete the remote documentation review. The inspector reviewed 1 child record and 6 provider home records. The executive director was interviewed during the inspection. An exit interview was conducted by telephone with the executive director and contractor on 8/26/2021 at 2:30 pm where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation: Based on interview with agency representative (AR) AR1, the licensee failed to send written notification of the decision to suspend the provider. Findings: AH6 is listed as being suspended on the list of approved provider homes requested for the inspection. During an interview with agency representative (AR) AR1 when asked for the written notification regarding AH6's suspension, AR1 acknowledged that written notification was not sent to AH6.

Description:	Violation: Based on review of background checks and sworn disclosure statement, the licensee failed to ensure a search of the child abuse and neglect registry maintained by any other state in which the prospective adoptive parent resided within the last five years was obtained prior to approval. Findings: The adoptive home (AH) record for AH1 contains a sworn disclosure statement for P1 in AH1 that documents P1 has resided in state (S) S1 within the last 5 years. The record for AH1 does not contain the results of a search of the child abuse and neglect registry maintained by S1 for P1. An email received on 08/26/21 from agency representative (AR) AR1 confirmed, that the licensee failed to complete the out of state child abuse and neglect registry check for P1.
