An unannounced renewal inspection was conducted on February 2, 2022 from 10:00 am to 2:45 pm for the office of Catholic Charities of Eastern Virginia (CCEVA), in Chesapeake, Virginia. The inspection was conducted from the Chesapeake office of CCEVA due to Norfolk records being stored at the Chesapeake location. This office reports 21 approved adoptive homes and no children are placed. The executive director, director of family engagement and adoption specialist were present for the inspection and exit interview which commenced on 2/2/2022 at approximately 2:00 pm. During this inspection, three adoptive home (AH) records were reviewed. Background checks for one employee and the four corporate board officers were reviewed. Three agency representatives were interviewed during the inspection. There was one citation for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on review of the contacts for Adoptive Home (AH) AH2 and AH3, the licensee failed to ensure the provider home was monitored every 90 days. Findings: AH2 was approved on 10/10/2017 and AH3 was approved on 8/20/19. AH2 and AH3 do not have any children placed. The record for AH2 documents contacts, dated, 6/24/21 and 10/4/21-beyond every 90 days. The record for AH3 documents contacts, dated, 6/8/21 and 10/4/21-beyond every 90 days. During an interview with the adoption specialist, it was acknowledged that the contacts were out of compliance.
