Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 4/4/2023 from 9:30 to 1:30 p.m. The Licensing Inspector met with the Director of Adoption Services to discuss the entrance to the inspection. Number of provider home records (9) Number of provider home records reviewed (1) Number of staff records reviewed (1) The agency has hired (1) new staff since that last inspection. The physical office was inspected and is in compliance with the standard 22VAC40-131-50. The agency's license and results of the most recent licensing inspection were posted near the agency's entrance, in compliance with 22VAC40-131-50. An exit meeting was conducted with the Director of Adoption Services on 4/10/2023. The evidence gathered during the inspection determined that there was (1) violation with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. Description:	Violation: Based on the record review, the agency failed to document that the home complies with standards for the home as required by 22VAC-40-131-190. . 1)The agency?s own CCEVA home environment checklist form for (AH-1) was not completed, but signed by the adoptive parents. 2) The Licensing Specialist asked (AR-2) if the information could be located. 3) The findings were discussed during the preliminary findings review and the staff acknowledged the findings. 4) The findings were reviewed during the exit interview.
