Type of inspection: Monitoring A monitoring inspection was initiated on 6/3/2024 and concluded on 6/7/2024. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census was (2). The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed (1) resident record, (2) staff records, (2) foster/adoptive home records, all background related documentation per remote protocol checklist and agency standards submitted by the facility to ensure documentation was complete. An exit interview was conducted with the Executive Director and CEO on 6/7/2024, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
