Type of inspection: Monitoring

Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 11/7/2024 from 11:30 p.m. to 2pm.

The Licensing Inspector met with the Executive Director of Adoption Services to discuss the entrance to the inspection. The Virginia Beach office conducts child placing activities and only houses the adoptive home and birth parent/child records. The staff conducting the child placement activities records are stored at the Norfolk license location and were reviewed on 11/8/2024.

The agency has not hired any new staff since that last inspection.
The physical office was inspected and is in compliance with the standard 22VAC40-131-50. The agency's license and results of the most recent licensing inspections were posted near the agency's entrance, in compliance with 22VAC40-131-50.

An exit meeting was conducted with the Executive Director of Adoption Services and Chief Program Officer on 11/19/2024. The evidence gathered during the inspection determined
that there was (1) violation with applicable standard(s) or law.

Standard #: 22VAC40-191-40-D-1-c
Description: Violation:
Based on review of adoptive home record, agency failed to complete the sworn statement, central registry findings and criminal history record check before three years since the dates of the last sworn statement, central registry and criminal history record check was completed.

Findings:
1. Adoptive home #1 was approved on 12/29/2021 and sworn statement, central registry findings and criminal history record check was not renewed within three years from the last date of completion.

2. At time of inspection, there was no documentation of a current sworn statement was completed before three years since the date of the last sworn statement. Last sworn statement in AH#1 record was completed 9/30/21.

3. At time of inspection, there was no documentation of a current central registry findings report was completed before three years since the date of the last central registry check. DSS Central registry findings in AH#1 record was dated 11/5/21 and 11/6/21.

4. At time of inspection, there was no documentation of a current criminal history record check was completed before three years since the date of the last criminal history record check. Criminal history record check in AH#1 record was dated 10/5/21.

5. The findings were reviewed during with agency representatives during exit meeting conducted 11/19/2024 and staff acknowledged inspection findings.

Plan of Correction: In respect to the violations of completing the sworn statement, central registry and criminal history check, the agency will implement a review of state standards for all unit employees. The unit will conduct quarterly audit checks of parent?s records and as well as spot audits conducted by the director. This process will ensure that in the future no documents required for approval will be out of compliance going forward. All documents will be updated within the 36-month timeframe.
