An unannounced non-mandated inspection was conducted on January 31, 2020 from 9:40 AM - 2:20 PM. Hope Tree Family Services reports a total of 8 provider homes and 8 foster children in placement. During this inspection, one provider home record and one foster child record was reviewed. One personnel was interviewed during the inspection. An exit interview was conducted at the conclusion of the inspection with the Director of Foster Care and Adoption - Western Region (by phone) and the Foster Care Supervisor to review preliminary inspection findings. An acknowledgement form was signed. There was one violation.
The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Standard #: 22VAC40-131-190-L Description: Violation:
Based on a review of the records for a provider home (PH1) and foster child (FC1) and interview with personnel (S1), the agency failed to ensure the bedroom for FC1 placed in FH1 had a door for privacy.
Evidence:
1. During an interview with personnel, S1 reported the agency had discovered that the bedroom FC1 occupied at PH1 did not have a door for privacy.
2. FC1 was moved to a different room than the one occupied at placement in FH1.
3. S1 stated that the agency was aware of the move but unaware that the new room for FC1 in PH1 did not have a door until it was brought to their attention by an outside agency.
4. The record for PH1 and FC1 contained an incident report which stated the agency had become aware that FC1 had been in a bedroom in PH1 which did not have a door for privacy as required y 22VAC40-131-190.LPlan of Correction: When the report was received that there was no door on the foster child's bedroom, the foster child was moved immediately and the foster home was suspended. Future monitoring visits will be made and Caseworkers will ensure/verify all standards are met regarding the foster child's bedroom. No other placements will be made in the foster home until the standards are met and the foster family is taken off suspension.
