Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 1/30/2022 from 10:30a.m. to 4:00 p.m.
The Licensing Inspector met with the Associate Director of Foster Care and the Foster Care Case Worker (AR-3) met at the site to start the inspection.
Number of approved provider homes: (14)
Number of children in care: (8)
The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: (2). Number of provider home records reviewed: (1).
Number of staff records reviewed: (1) new staff reviewed during this inspection. Observations by licensing inspector: posting of the current license and recent inspection. An exit meeting was conducted on the date of the inspection. There were no changes made to the policies or procedures since the last inspection.
The Licensing Specialist interviewed the Foster Care Case Worker (AR-3).
The evidence gathered during the inspection determined (2) violations with applicable standard(s) or law.

Standard #: 22VAC40-131-180-J-2
Description: Based on the review of the records presented for Foster Home FH-1 and based on an interview with Agency Representatives (AR), the licensee failed to address other weapons owned (i.e., knives and sling shot) in the home study narrative.
Findings:
1 Based on the review of the licensee certified foster parent home study FH-1 dated 10/1/2022, there was no information in the home study narrative addressing the other weapons (knives and sling shot) owned.
2. The licensee?s own safety information form identifies other weapons owned (knives and sling shot), but these items are not included in the home study narrative.
3. During the exit interview AR-2 confirmed and acknowledged the absence of the information in the home study documentation.

Standard #: 22VAC40-131-290-I
Description: Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with Agency Representatives (AR), the licensee failed to list the medications for the child.

Findings:
1. Based on the review of the Foster Child (FC)-1?s child record, the child was prescribed medication on 11/2/2022.
2. The face sheet dated 12/12/2022 failed to include the child?s medication prescribed on 11/2/2022 by the Nurse Practitioner
3. The monthly report update dated 12/2022 did not include a list of the medication.
3. During the exit interview, AR-1 confirmed and acknowledged the absence of the information.
