Comments:

An unannounced renewal inspection was completed on February 19, 2020 from approximately 10:00 a.m. to 3:40 p.m. and on February 20, 2020 from approximately 10:00 am to 12:30 p.m at the Chester office. At the time of the inspection, 23 children were in care and 39 approved provider homes. During this inspection, three foster child records, four provider home records and four personnel records were reviewed. Background checks for the board officers and the regional director of foster care and adoption were reviewed on 02/10/2020 at the Salem office. An Acknowledgement form was left with the associate director of foster care and adoption on February 20, 2020. There was one citation for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department's public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has ten calendar days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on a review of the record for Foster Child (FC) FC1 and interview with the assoicate director of foster care and adoption, the licensee failed to include the name of previous caretaker(s). Findings: FC1 was placed with the agency on 9/24/19. The social history completed by the licensee documents previous foster care placememt for FC1. The social history fails to list the name of the previous caretaker(s). During an interview with the assoicate director of foster care and adoption it was acknowledged that the information was not included in the social history.
