Discussed the responsibility of financial payments, including method of payment, frequency of payment, and amount of payment as it pertains to the written agreement for participation as noted in the ?Standards for Licensed Child Placing Agencies? ? 22 VAC 40-131-440.E.5.

Type of inspection: Monitoring

Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
1/11/23 from 9:50 AM ? 4:57 PM

The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

Number of children in care: 7
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
Number of youth records reviewed: 1
Number of staff records reviewed: 4
Number of interviews conducted with staff: 3
Additional Comments/Discussion:
An entrance conference was held at the agency on 1/11/23. Also, on the same date, a youth?s record was reviewed at the
agency. The Site Supervisor was available and accessible during the inspection. The Site Supervisor was interviewed on the same date with the Program Manager (by phone) to address questions about documents reviewed in the youth?s record. Staff records were also reviewed while at the agency. The Human Resources Manager was interviewed separately by phone in regard to a staff record. An exit meeting was conducted on 1/18/23 to review the inspection findings.

The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.

The department's inspection findings are subject to public disclosure.
