Type of inspection: Monitoring

Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
10/11/23 from 10:42 AM ? 4:54 PM

The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

Number of youth in care: 6
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
Number of staff records reviewed: 2
Number of interviews conducted with staff: 2
Additional Comments/Discussion:

An entrance conference was held at the agency on 10/11/23. Also, on the same date, a youth?s record was reviewed at the
agency. The Live-In Case Manager was available and accessible during the inspection. A preliminary findings meeting was held on the same date. The Live-In Case Manager was interviewed with the Program Manager (by phone) to address questions about documents reviewed in the youth?s record. Staff records were also reviewed while at the agency.

An exit meeting was conducted on 11/1/23 to review the inspection findings.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Standard #: 22VAC40-131-150-B-1
Description: Violation: Based on an interview with staff and a review of staff?s, S2?s, personnel record, the agency failed to ensure that S2, who has the responsibility to work with children and their families, received orientation prior to assuming the responsibilities of his position in the licensee's child-placing agency and within 30 days of the date of employment.
Findings:
1) As noted in the personnel record, S2?s date of employment is 8/28/23.
2) According to the following form - ?Staff Development Form ? First 30 Days,? the training elements required for orientation in relation to this standard were all completed on 9/29/23.
a. The above-mentioned completion date is actually 32 days after the date of employment.
b. The total of 32 days is based upon the following calculations: 8/29/23 ? 8/31/23 totals to 3 days and 9/1/23 ? 9/29/23 totals to 29 days.
3) Staff, S3, who signed the document as S2?s supervisor, acknowledged the Licensing Specialist?s findings.
