Standard #: 22VAC40-131-150-B-1 Description: Violation: Based on an interview with staff and a review of staff?s, S2?s, personnel record, the agency failed to ensure that S2, who has the responsibility to work with children and their families, received orientation prior to assuming the responsibilities of his position in the licensee's child-placing agency and within 30 days of the date of employment.
Findings:
1) As noted in the personnel record, S2?s date of employment is 8/28/23.
2) According to the following form - ?Staff Development Form ? First 30 Days,? the training elements required for orientation in relation to this standard were all completed on 9/29/23.
a. The above-mentioned completion date is actually 32 days after the date of employment.
b. The total of 32 days is based upon the following calculations: 8/29/23 ? 8/31/23 totals to 3 days and 9/1/23 ? 9/29/23 totals to 29 days.
3) Staff, S3, who signed the document as S2?s supervisor, acknowledged the Licensing Specialist?s findings.Plan of Correction: Not available online. Contact Inspector for more information.
