Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: Entrance conference, inspection of the office setting, records reviews, interviews, and a preliminary findings review were completed on 7/25/2022 10:00 a.m. to 2:35 p.m. An Acknowledgement of Inspection form was completed and provided to the agency. An exit interview was conducted by phone on 7/26/2022. Written Summary and Supplemental Forms were provided to the Treatment Foster Care Director prior to the exit interview. Number of children in care: 3 Number of approved provider homes: 5 The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
