Technical Assistance provided regarding standard 22VAC40-131-260-B.12 format for including information in the social history from previous placements.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on May 24, 2021 and concluded on June 2, 2021. The Therapeutic Foster Care Program Director was contacted by email to initiate the inspection. The agency reported 19 children in care, and 19 approved provider homes. The agency reports 2 new staff since the last inspection. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 complete child record, 2 complete provider home records, 6 additional provider home records (background records only) and 2 newly hired staff records submitted by the agency to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Based on the review of the file for Foster Homes (FH) 1thru 8 and based on an interview with Agency Representatives (AR), the licensee failed to document the title of the individuals approving provider homes. Findings: 1.Foster Home Certificate of approvals for FHs 1-8 did not contain the title of the individuals approving the homes. 2.During the exit interview, AR2 reviewed the certificate and acknowledged the absences of the title of the individual approving the homes. 3.During the exit interview, AR1 acknowledged the absences of the title of the individual approving the homes.

Findings: 1.FC-1 record did not contain documentation of a medical examination that demonstrated evidence of freedom from communicable disease, including tuberculosis. 2.During the exit interview, AR2 reviewed the available medical examination and acknowledged the absence of a medical examination that demonstrated evidence of freedom from communicable disease, including tuberculosis.

Description:	Based on the review of the file for Foster Child (FC) 1 and based on an interview with Agency Representatives (AR), the licensee failed to demonstrate evidence of the development of an individualized service plan within 14 days of the child?s placement. Findings: 1.FC-1 date of placement is 12/16/2020; the initial individualized service plan submitted for review has a date of 01/15/2021. 2.During the exit interview, AR2 reviewed the record and acknowledged that there is no evidence of the development of an individualized service plan within 14 days of the child?s placement.
