63.2 (1) General Provisions

Technical Assistance:
22VAC40-131-260-Social History-discussion regarding lack of detail in the social history reports and discussion regarding the format used for the completion of social history reports.

Comments:
A renewal inspection was initiated on July 6, 2021 and concluded on July 12, 2021. The Program Director was contacted by telephone to initiate the inspection. The Program Director reported that the current census was 19 foster children, 22 active foster homes and 1 newly hired staff. The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 2 foster child records, 3 foster home provider records and 1 personnel record submitted by the facility to ensure documentation was complete. Board Officer records were previously reviewed. An exit interview was conducted with the agency representatives on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-131-250-M
Description: Based on the review of the file presented for Foster Child (FC) #2 and based on an interview with Agency Representatives (AR), the licensee failed to arrange a preplacement visit for FC #2 in the prospective home.

Findings:
1. Based on the review of the files submitted for FC #2, there was no documented preplacement visit for this foster child.
2. The files documented that this was not an emergency placement.
3. During the exit interview, AR#1 acknowledged that the youth was not an emergency placement. During this interview AR#1 acknowledged that there was no preplacement visit completed due to the child being previously placed in the foster home during a prior admission.

Standard #: 22VAC40-131-260-B-10
Description: Based on the review of the file presented for Foster Child (FC) #2, the licensee failed to document the child's emotional or psychological problems.

Findings:
Based on the documentation that was presented, this section of the social history was left blank.

Standard #: 22VAC40-131-260-B-6
Description: Based on the review of the file for Foster Child (FC)#1 and FC#2, the licensee failed to include documentation regarding the child's developmental history.

Findings:
1.Based on the documentation presented for FC#2, the social history documented "there are no developmental needs at this time" and did not discuss the developmental history.
2. Based on the documentation presented, the social history for FC#1 stated "no developmental history was provided.
