Technical Assistance:
The Licensing Inspector did provide technical assistance regarding the use of white out on agency documents and regarding the adding of more detail in the child?s emotional and psychological section of the social history.

Comments:
Type of inspection: Renewal
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 7/25/2022 from 10:30a.m. to 3:45p.m. and 7/26/2022 from 9:00 to 12:30 p.m. The Licensing Inspector met with the State Director and the Program Supervisor at the site to initiate the inspection.
Number of approved provider homes: (20)
Number of children in care: (20)
Number of children?s records reviewed: (2) and (2) discharged children?s records. Number of provider home records reviewed: (2) and (1) foster home background checks only.
Number of staff records reviewed: (4) and the (4) board officers. Observations by licensing inspector: posting of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection. There were no major changes made to the policies or procedures during the licensing period.
The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. The Licensing Inspector did provide technical assistance regarding the use of white out on agency documents and regarding the adding of more detail in the child?s emotional and psychological section of the social history.

Description: Violation
Based on a document review, the agency
failed to obtain sworn disclosures and central
registry searches for Board Officers 1 and 2 (B1 and
B2), before three years since the date of the
last background checks.
1) The Licensing Inspector requested and
reviewed the sworn disclosures, central registry, and
criminal background results for B1 and B2.
2) The sworn disclosures for B1 was dated 4/17/2019 and B2
dated 5/16/2019
3) The central registry search results were
dated 5/7/2019 for B1 and 5/17/2019 for B2.
4) The findings were discussed during the
preliminary findings review and the Licensing Inspector has
requested copies of the updated checks from
the Program Supervisor.
5) The findings were discussed during the exit
interview and A1 and A2 acknowledged the findings.
