Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 6/30/2023 from 10:45 a.m. to 3:05 p.m. The Licensing Inspector met with the Clinical Supervisor, and Foster Care Supervisor at the Richmond site to review electronic records for the Lynchburg site. Exit interview was conducted on 6/30/2023 at the Richmond Office. Number of approved provider homes: (20) Number of children in care: (15) Number of children?s records reviewed: (1) and (1) Discharge record. Number of provider home records reviewed: (2) Number of staff records (2) Technical assistance was provided regarding transitional plans for youth and aftercare recommendations and written discharge summaries. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law.
