Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 3/19/2024 from 10:30 a.m. to 4:05 p.m. The Licensing Inspector met with the Program Supervisor. Exit interview was conducted and concluded on 3/19/2024 at the Forest Office. Number of approved provider homes: (21) Number of children in care: (24) Number of children?s records reviewed: (2). Description:	Violation: Based on the record review for FC-1, the agency failed to complete an intake assessment containing all required elements specified in 22VAC40-131-250 G. Findings: 1. The initial intake assessment documentation dated 8/24/2023 in the record for FC-1 did not contain all the required elements as specified in 22VAC40-131-250 G. 2.The findings were discussed during the preliminary findings and exit interview

Description:	Violation: Based on the foster parent record review for FH-1, the agency failed to complete an out of state central registry check as required by the background check standards for child welfare agencies. Findings: 1. The sworn statement for FH-1 (Individual #1) dated 1/5/2023 indicated that the individual resided in the state of Rhode Island within the last five years. 2. The agency failed to conduct a central registry check from the state of Rhode Island for Individual #1. 3. The findings were discussed during the preliminary findings and exit interview.
