Comments:

An announced initial inspection was completed on December 31, 2019 from 10:00 am - 1:30 pm and a follow-up desk review was completed on January 8, 2020 from 11:00 am - 12:15 pm. This is an initial inspection in response to a new application for licensure. In compliance with licensing requirements for an initial application, the background checks for corporate board officers and agents of the corporation, personnel records, the agency's policies and procedures and the agency's program statement were reviewed. The physical office was inspected and is in compliance with 22VAC40-131-50. The President, Chief Operating Officer and Child-Placing Supervisor were available for the inspection and interview. An exit meeting was held at the conclusion of the onsite inspection. An acknowledgement form was provided. There were no areas of non-compliance as a result of this initial inspection. The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.
