Comments:

An unannounced monitoring inspection was conducted on February 9, 2022 from 11:34 a.m. to 3:13 p.m. At the time of the inspection the agency reports seventeen (17) youth placed in independent living arrangements. During this inspection, two (2) personnel records and two (2) youth records were reviewed. An acknowledgment form was signed. An exit meeting was conducted with Executive Director on February 9, 2022, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.
