Type of inspection: Monitoring An unannounced, mandated monitoring inspection was completed on 4/23/24. The inspector was on site at the Roanoke facility for inspection of files from 12:45 p.m.? 1:15 p.m. A preliminary findings review and exit was held on this date with the Regional Director. An acknowledgement of inspection form was signed and left at the facility. A site visit at the Lynchburg location was completed on 3/27/24. Number of children in care: 0 Number of approved provider homes: 1 The licensing inspector completed a tour of the physical plant that included the office setting and conditions on 3/27/24. Number of children?s records reviewed: 0 Number of provider records reviewed: 1 Number of staff records reviewed: 0 Number of interviews conducted with staff: 1 Additional Comments/Discussion: The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
