Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced initial inspection was initiated on 5/6/2021 in response to a new application for licensure. The inspection concluded on 5/17/2021. A desk review of the applicant?s policies and procedures, program statement, and personnel policies was completed. A remote site inspection was completed on 5/17/2021 and the office space was in compliance with 22VAC40-131-50. Background checks and documents required from personnel records were reviewed for all staff identified to serve the program.

Preliminary findings from the inspection were discussed during an exit interview on 5/17/2021.
There was one (1) citation for a Violation of the Standards for Licensed Child Placing Agencies. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on a review of personnel documents for personnel 4 (P4), central registry search results were not available.

Findings:
1) A central registry search result letter for P4 was provided.
2) The last four digits of the social security number of P4 did not match the information in the central registry search results letter.
3) The findings were discussed with personnel 6 (P6). P6 acknowledged the finding.
4) P6 is coordinating with the applicant?s Human Resource staff to ensure that the central registry search is conducted and a results letter is obtained for P4.

Plan of Correction: In response to the violation identified in the review of personnel records, on May 17, 2021, specifically the mismatch of the last four Social Security digits for one employee on the Central Registry response letter. HCS will seek correction from the central registry on 5/18/21. HCS will coordinate this effort through its Human Resources Department to ensure the letter is obtained and made part of the employee?s personnel record. A copy of the corrected letter will be sent to you upon receipt.
