Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 1/31/2024 from 11:00a.m. to 2:30 p.m. The Licensing Inspector met with the Executive Director and the Intake Specialist at the license office location to initiate the renewal inspection. Number of approved provider homes: (0) Number of children in care: (0) Number of children's records reviewed: (0). Number of provider home records reviewed: (0). Number of staff records reviewed: (8). Observations by licensing inspector: Posting of the license and inspection documentation was observed. Provider is currently not serving individuals in the Independent Living arrangement program. Licensing specialist conducted a physical plant inspection of the interior and exterior of the two locations being utilized for the Independent Living arrangements. Licensing inspector reviewed eight employee records to include background results and documentation of employee?s tuberculosis screening. Employee records were well maintained and in compliance with DSS standards during this renewal inspection. An exit meeting was conducted on the date of inspection with the Executive Director and Intake coordinator and an acknowledgement form was provided. There were no major changes made to policies and procedures since initial inspection and the ages of individuals being served were updated in agency?s program description. A meeting was scheduled and completed with provider, Licensing administrator and Licensing specialist to discuss renewal requirements, mandated inspections and license types on 1/31/24 at 4:00 p.m. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
